What are your tips for increasing PPO insurance reimbursements and other keys for ensuring financial and operational success?

Dr. Michael DiCarlo

Holmdel, Monmouth Beach, and Toms River, New Jersey

"The cost of labor and goods is constantly increasing and we need to make sure we stay on top of these price hikes, which eat into our bottom line, especially in today's inflationary environment. Our offices run a practice/insurance analysis report that allows us to analyze our current fees and reimbursement rates from insurance companies on an annual basis. Many offices and practice owners aren't aware of these services and they tend to miss out on higher fees and increased reimbursement rates from insurance companies. I recommend raising office fees on an annual basis. We also use a company to assist in renegotiating fee schedules in our offices that participate in PPOs. We had a positive experience and have seen reimbursement from insurance increase as much as 20%. As a result, we have had a significant increase to our bottom line."

"Insurance carriers are in a constant state of change, especially in the current climate of the pandemic and inflation. Because of this, it is critical to ensure you have an open line of communication with the insurance carriers you are contracted with. There are methods to potentially increase reimbursements, but the most effective is understanding how and when to negotiate with the carriers. Your current office fees, last negotiated rate and length of service will factor into this negotiation process. In addition, it's important to understand the different types of insurance carriers you are contracted with (do you have a direct contract or is there a middle-man involved that's taking a percentage of your fees?). There's no precise science to this, but it will require an ample amount of time to properly analyze and understand what is best for your practice, which most dental practices often don't have unless they employ a seasoned/experienced office manager to handle the administrative heavy lifting. Seeking out a third-party vendor to negotiate on your behalf can be very valuable in the long run."

"We do not participate in PPOs, however, [to help improve our bottom line], we involve the entire team in our WHY. We do profit-sharing quarterly, which contributes to increased production. We also talk about how overhead impacts profit-sharing, and try to identify want versus need. From there we make team decisions."

Nina Harris

"The main focus for increasing quick and accurate reimbursement is the claim submission. We make sure the correct insurance information is attached to the patient, and then submit the claim electronically, attaching all necessary x-rays and comments in the remarks section of the claim. When submitting for scaling and root planing (SRPs), we're able to quickly upload perio charting along with x-rays. As a result, we're paid right away."

Dr. Al Ochoa

"I strongly believe that every dental practice should ask themselves four key questions: how many patients do I treat each month?; how much profit do I make per procedure?; what is my average patient value (total production divided by total patients treated)?; and how can I increase each of these three numbers? There are countless services, and companies, such as Unitas, that can help address these responses and find resolutions. As doctors, we need to focus on which area we want to improve first, and then find the right experts to help us achieve those goals."

Dr. Terry Yacovitch
Dr. Terry Yacovitch

Yacovitch Dental

"To help improve the bottom line, explore additional practice-enhancing treatments, such as sleep dentistry; get proper training to deliver more specialized dentistry, such as sleep and laser treatment; invest in sleep analysis equipment and offer courtesy discounts for these sleep analyses; develop an open relationship with a cardiologist, respirologist, or pneumologist for medical support as they diagnose based on data, and then you can treat them; and free up doctors' time by delegating responsibilities across the whole team."
