All in-kind donation programs require that you satisfy the following requirements (plus individual programs may have additional requirements):

• Satisfy one or both of the following: (i) be a 501(c)(3) non-profit organization and be able to provide a copy of your IRS Determination Letter and/or (ii) be a Health Care Provider providing free indigent care
• Recipients must execute a donation agreement with Henry Schein and adhere to the terms
• Recipients need to disclose timing, scope, and population in the application as these donations are generally intended for events of limited scope and duration
• Recipients are required to provide an impact report upon completion of their project
• Donations should not be used for raffles, sweepstakes, or any other type of giveaway
• These donations must serve the public without discrimination on the basis of race, color, creed, sex, religion, age, disability, sexual orientation, marital status, gender identity, national origin, or any other protected category
• Ensure that recipient's organization and program facilities are in compliance with the Americans with Disabilities Act (if relevant)
• These donations must not require articulation of any religious or political belief, the submission to religious or political counseling, or the attendance at any religious or political activity
• The program for which the donation is used must not aid or support direct religious activities, political organizations or candidate's fundraising efforts, financial deficits or as a replacement of public sector funds, or political lobbying (beyond the lobbying allowed by law for non-profit organizations)
• Recipients must make good faith efforts to reduce your environmental impact

• The date you need to receive donation by should be at least 6 weeks from the date of application submission to allow adequate time to process your request. Applications inside that time frame may not be fulfilled on time
• You may be asked to provide supporting documentation, including (but not limited to): IRS Determination Letter, most recent 990 tax form, Operating Budget information, Annual Report, Letter of Recommendation, etc. Please be prepared to attach these documents to your application or provide them upon request

The Henry Schein Cares Package is a series of six kits geared toward health care professionals to Help Health Happen. The six packages are categorized as:

We worked with subject matter experts (SMEs) across the health care profession to create the list of materials included in each package.

All products donated by Henry Schein Cares must be used only for indigent patients and must be distributed free of charge. These products may not be resold and no fee (not even a nominal fee) will be charged to the patient for the in-kind donated products or services utilizing the in-kind products where the product cost would be a component of such fee.

No. We only are able to ship within the United States. If you are taking the product out of the United States, you are responsible for the importation and distribution, including, but not limited to, obtaining any required product registrations or licenses (as may be applicable), or exemption letter from in-country Ministry of Health, of the donated in-kind products in accordance with applicable law.

It is your responsibility to ensure that products donated by Henry Schein Cares will be exported from the United States in strict compliance with all relevant U.S. laws and regulations, including but not limited to, obtaining and providing any necessary export licenses for countries that require them and ensuring that no product will knowingly be provided to any sanctioned, denied party, individual or entity that commits, attempts to commit, advocates, facilitates, or participates in terrorist acts. When we process your shipment, we will provide you with a manifest, listing the donated products and a letter stating that they have been donated for charitable use only. If products are confiscated by the airline or customs, please notify us as soon as possible.

Any excess donated products remaining at the end of your project should be distributed to local health care facilities with qualified health care personnel. Products should be distributed to patients prior to expiration; expired products should not be distributed and should be properly destroyed. No products should be sold or exchanged.

Yes. Because our donations are project-specific, we need a new application for each request. Our online application includes a login/password feature that will allow you to save your basic profile information.
