RETURNS 

All returns should be sent to: Zahn Dental Co., Inc., 41 Weaver Road, Denver, PA 17517

  1. Items must be returned complete, in the original container, and adequately cushioned to prevent damage in shipping.
  2. Defective items are repaired or replaced under the manufacturer's warranty terms and conditions.
  3. Returns must be sent by traceable carrier (e.g., UPS).
  4. Return freight costs are the responsibility of the customer.
  5. Returns not following the above policies will be refused or subject to a restocking charge.
  6. Returns meeting all of the above policies receive the appropriate credit or refund.

Teeth:

  1. All tooth returns must include customer name, address, phone number and account number. Be packed tightly (in tooth boxes with foam on top of teeth) and sent by traceable carrier (e.g., UPS).
  2. Full sets of teeth, on the original tooth cards and unground.
  3. You may return up to 30% of your prior month’s purchases.
  4. Nonreturnable items include: Gold Crowns, and Henry Schein Brand Teeth, Porcelain Teeth, Dentsply lines Portrait and Classic, teeth on the incorrect block, broken blocks, damaged wax, discontinued items, shade guides and teeth that are unclean, ground, uncarded or miscarded.

DELIVERY TERMS

MERCK DROP-SHIP ITEMS

All Merck drop-ship items are non-returnable. Merck ships their products by second day service unless overnight delivery is requested (subject to an additional charge). ADDITIONAL TERMS AND CONDITIONS

All sales transactions shall be deemed to be fully negotiated, transacted and performed within the County of Bland, Commonwealth of Virginia, U.S.A. All sales transactions and sales agreements shall be governed by and construed according to the laws of the Commonwealth of Virginia. In the event of any dispute concerning a sales transaction or sales agreement, including, without limitation thereto, Customer’s obligation to pay for goods sold by Henry Schein, the proper forum for legal or equitable claims and/or proceedings shall be either the General District Court of Bland County, Virginia, USA, or the Circuit Court of Bland County, Virginia, U.S.A., and to this end, Henry Schein and Customer agree to subject themselves to the jurisdiction of said court in the event of any such dispute. Henry Schein hereby reserves the right to amend these Terms and Conditions at any time.

ADDENDUM TO RETURNED GOODS POLICY AND SHIPPING DOCUMENTS

In accordance with Internal Revenue Code Section 4132(b)(3), the manufacturer of vaccine products must establish that it has repaid or agreed to repay the amount of the federal excise tax to the ultimate purchaser of the vaccine before it may receive a credit or refund of excise tax paid on returned or destroyed products. In the event that Henry Schein's Customer is not the ultimate purchaser of the product, Henry Schein's Customer hereby certifies that all refunds of excise tax are, and shall be, paid to the ultimate purchaser of the product.

When a 340b Participant utilizes a third party return goods company for the return of non-saleable inventory, Henry Schein will, subject to its standard terms and practices, work with such company to pass through credits to 340b Participant with no handling charge imposed on 340b Participant by Henry Schein.

At your instructions, orders can be shipped by International Air Parcel Post, Surface Parcel Post, Air Cargo, Ocean Cargo, FedEx, UPS International or by ScheinDirect™.

Electrical equipment shown is designed for U.S. voltage (110 volt). Some of these items are available in 220 volt / 50 cycle. Please contact our International Department and we will be glad to provide more information (see below).

All prices in our catalog are quoted in U.S. dollars, f.o.b., Indianapolis, Indiana. We request payment in U.S. dollars by any of the following means:

  • Use your American Express, Visa, or Mastercard.

A bank charge of $75 (U.S.) for processing the Letter of Credit will be added to the order. Please contact the International Department for detailed terms BEFORE issuance of the letter of credit.
  • No class drugs can be sold.

Hazardous materials: We are able to ship small quantities of low-level hazardous materials to the following countries: Barbados, Belgium, Channel Islands, England, France, Germany, Holland, Ireland, Israel, Italy, Japan, Korea, Liechtenstein, Luxembourg, Monaco, Netherlands, Philippines, Scotland, St. Maarten, St. Martin, Switzerland, Tobago, Trinidad, United Arab Emirates and Wales.
